The Upcounty Hub's Diaper Distribution Program serves children ages 0-3.

If a child over the age of 3 requires diapers due to a medical condition or special needs, a note from their medical provider will be required.

We offer diaper sizes ranging from 0 to 6, but please note that we do not carry pull-ups.

​

We do not require proof of income.

 

To be eligible, applicants must provide proof of identity and address for the parent or guardian, as well as proof of the child's age.

Applications can be submitted in person via a paper form or online through our Microsoft Form.

​

Once the application is completed, applicants will receive a phone call with their appointment date and time for diaper pick-up.

ELIGIBILITY REQUIREMENTS:

Documentation Required for the Diaper Distribution Program:

• Proof of identity 

• Proof of address for the parent or guardian

• Proof of age for the child (medical insurance card or birth certificate)
